a credit card auth form (will hold registration 7 days from submission) or
an online payment.
Be REMINDED: Registration must include Regsitration Fee(s) .
Classes fill very quickly, and your position cannot be confirmed until we receive your registration form and payment.

I, the undersigned, understand that Jennifer Kroger & BellaBing,Inc. / Art from the Heart staff are not responsible in any way whatsoever for any accidents or injuries that may occur while attending classes, or on studio property at any time. I also understand that this disclaimer applies to any out of class performances being held. As such, I understand that no monetary compensations can be sought from either Jennifer Kroger or the AFTH staff related to previously mentioned accidents or injuries. By signing, I also accept the terms and conditions of payments, as stated being due the 1st class of each month, with a $15 late fee per student being incurred for late payments, and will abide by all established rules / procedures. I agree that I have read, understand and agree to all facility rules and requirements, tuitions, and payments policy and procedures. I also understand that any non-compliance with the above could result in my child's removal from AFTH.
